I agree to abide by the club rules and code of conduct and give permission for the above-named child to participate in all activities at LX Gymnastics.

I am aware that fees are due before the commencement of each term.

I understand that my child must become a member of British Gymnastics to ensure they have adequate insurance to take part in their class.

If my child should miss 3 weeks or more I will inform the coach and understand that failing to do so could result in the loss of my child's place, I understand and accept that no refund will be given for any reason of absence or termination of my child's membership.

I acknowledge that, as with all children's sports, participation in gymnastics classes carries a certain risk of injury. I understand that LXGC conducts risk assessments for all activities, and implements all relevant mitigations to reduce the risk of injury.
